Job Description

CVS Health is seeking a dedicated and skilled Remote Nurse Consultant to join our dynamic team. Requirements

• Active RN license in the state of residence


• Bachelor’s degree in Nursing (BSN) preferred


• Minimum of 3 years of clinical experience


• Previous experience in case management or consulting roles is a plus


• Proficient in using electronic health records (EHR) systems


• Strong analytical and problem-solving skills


• Excellent verbal and written communication skills

Responsibilities

• Provide remote consultation and clinical guidance to patients and healthcare providers


• Conduct comprehensive health assessments and develop care plans


• Coordinate and manage patient care, ensuring continuity and quality


• Collaborate with interdisciplinary teams to optimize patient outcomes


• Utilize telehealth platforms to deliver services and monitor patient progress


• Educate patients and families on health management and treatment options


• Maintain accurate and up-to-date patient records in compliance with regulatory standards
